Conflicts of Commitment and Interest: Disclosure of an Outside Activity
Instructions
All University employees must disclose to their
Departmental Executive Officer (DEO) any activity that may create
a conflict of commitment or interest in the workplace as defined in the
University of Iowa Policy on Conflicts of Commitment and Interest in the
Workplace (UI Operations
Manual Part II, Chapter 18).
To the employee:
- Complete the information requested on the Disclosure of an Outside Activity form;
- Attach a written description of your outside activity that may constitute a conflict of commitment or conflict of interest in the workplace and describe the accommodations, if any, that you will make so that the activity does not interfere with your University obligations (if you have not planned any accommodations, state none);
- Sign the form;
- Give the form with your written disclosure to your DEO, keeping a copy of each for yourself.
- If the activity involves your professional expertise, report the activity on the Annual Disclosure of Outside Professional Activities form on the web. This web-based form is designed for you to report outside activities as they occur and save the form online until it is time to submit it (after June 30th each year). You will be notified each year when it is time to submit the form.
- If your DEO believes that your outside activities meet the University definition of conflict of commitment or conflict of interest in the workplace, s/he must develop a management plan and you will be asked to sign a management plan form indicating that you have been given a copy of the plan and agree to abide by it. (If you do not agree with the management plan, please see Operations Manual III-29.4 for steps to take to resolve the disagreement.) A copy of both your disclosure and the management plan will be provided to your Dean (or appropriate VP/Provost).
